We report a case of a 35 year old woman, who presented to casualty with left sided hemiparesis two weeks after the delivery of healthy twins by Cesarian Section. She had a history of hyperthyroidism. Supraventricular tachycardia and fever were noted together with a two week history of diarrhoea, weight loss, sweating and fatigue. MR imaging of brain showed an acute ischaemic stroke in the right gyrus precentralis. Further investigations into the cause of stroke in this young patient – such as cardioembolism, connective tissue disorder or thrombophilia – proved negative. The patient was treated with intravenous thrombolysis with complete clinical recovery. We presume that the most likely mechanism of stroke in this patient was cardioembolism caused by tachyarrhythmia in untreated thyrotoxicosis.
