Bosniak classification is currently the main diagnostic tool for the evaluation of complex cystic renal masses. The disadvantage of this classification system is that, only on basis of the radiodiagnostic criteria, it is really difficult to distinguish benign lesions from malignant ones. Also a significant scale of interpersonal variability adds to the diagnostic difficulty. The authors demonstrate a chronic renal abscess diagnosed as Bosniak IV complex cyst and show potential therapeutic modalities.
