A case of a 17-year- old female with anomalous origin of the left coronary artery from the main pulmonary artery (ALCAPA, Bland-White-Garland syndrome) is described. The patient had no sings of left ventricular dysfunction. The effort capacity tested by bicycle ergometry was very good. The diagnosis was stated by electrocardiogram, bicycle ergometry, echocardiography and angiography. The patient underwent surgery with good postoperative effect. Control testing after operation showed normal left ventricular function and no ischemic changes during bicycle ergometry.
