RISK FACTOR ANALYSIS OF THE ABNORMALLY INVASIVE PLACENTA IN CLINICAL MATERIAL

The abnormally invasive placenta represents rare and life-threatening complication of III phase of en_delivery. Throughout recent years occurrence of disorders of placental invasion has a rising tendency. 4037 births were analysed in this work (1st gynaecology and obstetrics clinic UHB and FMCU Bratislava year 2016/first quarter of the year 2017) in consideration of anamnestic detail presence (age over 35, multiparity, previous surgery on the uterus, especially after C-section, IVF) and actual risk factors for abnormal invasion of the placenta (placenta previa). In the achieved collection of 73 women, a correlation between actual occurrence of multiple risk factors and severity of the clinical picture was confirmed. Suspicion for abnormal invasion of placenta based on imaging methods (ultrasound, magnetic resonance) was compared with clinical and histopathological findings. A severe clinical course was documented in 9 cases, from which 8 patients required peripartal hysterectomy. Histologically 1 placenta accreta and 1 placenta increta was confirmed. It was documented that occurrence of several risk factors may correlate with a severe clinical course not only based on pathological invasion but also with not routinely used prenatal ultrasound diagnostics. For improvement in clinical practice it is required to change until now routinely used terminology of disorders of placentation, standardisation of processes in prenatal diagnostics when abnormal invasion of the placenta is suspected, mainly evaluation of ultrasound findings, and also more precise surgical steps planning.
