Amyloidóza tenkého čreva – kazuistika

Autori prezentujú kazuistiku 69-ročného muža, ktorý sa 14 rokov liečil na zváračské pľúca, mal bronchiektázie
a recidivujúce pneumónie. Mal diagnostikované rozsiahle adhézie a fibrotické zmeny pľúc. Bol prijatý na I. internú
kliniku pre nauzeu, vracanie, hnačky, chudnutie a malnutríciu. Rôznymi špecializovanými vyšetreniami bola
stanovená diagnóza amyloidózy tenkého čreva. Stav bol komplikovaný opakovanými krvácaniami z tráviaceho
traktu, až s nutnosťou podávania terlipresínu. Počas liečby terlipresínom vzniklo akútne zlyhanie ľavej komory
s pľúcnym edémom. Liečba a resuscitácia pacienta neboli úspešné, pacient exitoval. Patologická pitva potvrdila
sekundárnu amyloidózu nielen tráviaceho traktu, ale aj všetkých excidovaných orgánov. Príčinou smrti bol akútny
infarkt myokardu v teréne amyloidózy myokardu. Autori rozoberajú delenie amyloidózy, jej definíciu, charakteristiky
a komplikácie. Prezentácia je obohatená mnohými endoskopickými, histologickými a sekčnými nálezmi pacienta.

Amyloidosis of the small bowel – case report

Autors presents the case report of the 69 year old man suffered from welder’s lung, bronchiectasias and recurrent
pneumonia. He had extensive adhesions and fibrotic changes of the lung. He was admitted to the I.internal clinic
for nausea, vomiting, diarrhoe, weight losing and malnutrition. The amyloidosis of the small bowel was diagnosed,
the various specialized examinations were used. The state was complicated with recurrent bleeding from gastrointestinal
tract. Administration of the terlipressin and other complex therapy was required. The therapy was complicated
with acute cardiac failure and pulmonary oedema with death, in spite of resuscitation. An autopsy revealed
secondary amyloidosis not only gastrointestinal tract but the whole organs in the body. The cause of the death was
an acute myocardial infarction. of the amyloidosis affected heart. The authors analyze the definition, characteristics
and complications of the amyloidosis. Article contains endoscopic, histological and autopsy images.
