Right ventricle has its compensatory mechanisms to chronic overload which are to some extend specific in volume overload compared to pressure overload. Here is presented a case-report of chronic volume overloaded right ventricle firstly manifested by atrial flutter. Primary cause of this state was a rare form of partially anomalous pulmonary venous return - scimitar syndrome. Authors are analysing differential diagnostic tools in identifying the cause of chronic right heart volume overload where echocardiography and other imaging methods play the key role; but with the necessity of complex patient´s approach.
