Selective intrauterine restriction as a complication of originally ongoing so-called twin-anemia-polycythemia sequence in monochorial biamnial twins

Twin anemia-polycythemia sequence is a rare complication of monochorionic pregnancy. We report the case, in which repeated measurements and placental echogenicity difference confirmed the complication. According to the current classification dependent on the measurement of peak systolic velocity in the middle cerebral artery, we diagnosed the first and second stage of the sequence. Later in pregnancy, the difference in peak systolic velocity in the middle cerebral artery decreased with the development of mild donor’s anemia and its intrauterine growth restriction. Spontaneous closure of inter-fetal anastomoses and prior donor’s anemia throughout the pregnancy is a possible causal factor. Growth alteration was probably caused by fetal hypoxia induced by anemia leading to the macroautophagy. The sequence was confirmed by postnatal appearance of the placenta.
