Myelodysplastic syndrome (MDS) is a clonal disorder of haematopoietic stem cell characterized by ineffective and dysplastic haematopoiesis. The authors present review of pathophysiology of variable morphological features, chromosomal abnormalities as well as clinical manifestation of MDS. The work is focused on assessment of accurate diagnosis of MDS as well as importance of International Prognostic Scoring System, based on blast percentage in bone marrow, number of cytopoenias and cytogenetics that stratifies patients with MDS into 4 prognostic groups, with different survival probability and leukaemia transformation risk.
