Respiratory infections are the commonest therapeutic problem in clinical practice. Choice of antibiotics (ATB) should be regulated by clinical status of the patient and knowledge of the present state of resistance inducing pathogens. Benefit of macrolids and new respiratory fluoroguinolones in treatment of pneumonias can be explained by wide spectrum of their effectiveness and immunomodulating effect. Crucial factors in choice of a suitable empiric antimicrobial therapy of sepsis are detection of the basic cause - source of sepsis and spectrum of suspected pathogens. Patients with severe sepsis and septic shock require a complex therapy together with the most effective antiinfectives in intensive care units.
