Oesophageal carcinoma is one of the most problematic organ localisations of malignant tumors. The basic curative treatment of carcinoma of the oesophagus is surgical resection treatment. In spite of dramatical technical changes during last 50 years there are still some controversial opinions as to the effectiveness of the treatment ranging from pessimistic to optimistic ones. Opinions disunity results from the fact that 5-year surviving has been improved during last 20 years, but the value 20% , only in highly sophisticated surgical centers, is startling.The effort to improve surviving of patients lead to perfection of technical possibilities of correct preoperative staging of the disease, centralization of the treatment to higher centers, using off multimodal treatment and changes in technique and tactics of surgical treatment.
