Most frequently exocrine pancreatic insufficiency accompanies patients in advanced stage of chronic pancreatitis but it appears also in malignant gland destruction or in patients with diabetes. Mechanisms of endocrine insufficiency genesis are diverse, but the final effect of this condition is practically the same - the disorder of splitting and absorption of lipids and in lipid soluble substances, the disorder of starch splitting with the disorder of glucose homoeostasis. The final stage is pancreatic malabsorption. Besides these generally known facts, endocrine pancreatic insufficiency is related to influencing of motoric function of other organs, as e.g. the stomach, intestine or gall bladder. From this point of view the correct supplemental therapy by pancreatic enzymes is a fundamental measure as a treatment and therapy of dyspeptic condition induced by motility disorder in patients with endocrine pancreatic insufficiency.
