Ileus is a common diagnosis in routine clinical practice. Any doctor, regardless of specialization, can be confronted with this diagnosis and should be competent to manage it. The symptoms and severity of ileus depend on the location of passage disorder. Recently a patient suspected of mechanical ileus should have surgery within 12 hours; today ileus, especially of the small intestine, can be successfully treated conservatively. Likewise, the paralytic ileus only rarely requires surgery; supportive treatment is usually sufficient. The treatment of ileus depends on the early determination of the cause of the ileum and close interdisciplinary cooperation.
