Wegener´s granulomatosis is a systematic disease - a granulomatosis necrotizing vasculitis that has a predilection impairment of medium and small-sized vessels of airways and kidneys. We demonstrate clinical course of this disease in a 76-year-old polymorbid woman. As a part of the search for the causes of progressive renal insufficiency the presence for cANCA antibodies was tested. The positive results of the tests supported our suspicion for systemic vasculitis. Initiation of immunosuppressive treatment (prednisone and cyclophosphamide) was a key step which was followed by rapid improvement of clinical status, bronchopulmonary symptoms subsided, but patient remained hemodialysis-dependent. We highlight the need for ANCA testing as a part of differential diagnostic process and life-saving treatment with cyclophosphamid and prednisone even in polymorbid immunocompromised patients is demonstrated too.
