Differential diagnostics of single histological types of pancreatic cancer is not reliable from USG image. Chronic pancreatis can sometimes be complicated by occurrence of inflammatory or cancer focal infiltrations representing an important problem in differential diagnostics in CP. Problem is not in detection but in interpretation. To solve this problem of differential diagnostics EUS helps with target aspiration biopsy of the focus by a fine needle (FNAB). Supposing to get enough material for cytological or histological examination FNAB has 100 % specificity and quite good sensitivity. Indication of FNAB in solid focuses of pancreas: • Evidence of malignity in non-resectional mass as assumption of adjuvant chemotherapy/radiotherapy • Exclude lymphomas, meta small cellular carcinoma and neuroendocrine tumors treated by different strategy than pancreatic carcinoma • Refuse to undergo surgical resection procedure without knowledge of definite diagnosis • Support exclusion of malignity while preliminary probability of malignity is low Basic common indication for FNAB is that result will somehow affect further decision in diagnostics and treatment of the patient. FNAB controlled by EUS is a diagnostic procedure with a low frequency of complication up to 2 % (pain, infection, perforation, pancreatis, bleeding).
