The most frequent cause of early death with severe acute pancreatitis are cardiovascular collapse, acute respiratory distress syndrome (ARDS), acute renal failure, intra-abdominal hemorrhage. Severe acute pancreatitis is a prerequisite for the development of local complications: acute fluid collections, acute pseudo-cysts and pancreatic necrosis. Septic complications, particularly infected pancreatic necrosis and abscess account for the late mortality. The successful management of severe acute pancreatitis requires a team approach in intensive care unit. Treatment of local complications was traditionally surgical, but development of new endoscopic and laparoscopic methods gives us the possibility for more safe intervention on gastrointestinal tract.
