Endoscopic treatment and biliary decompression are efficient tools in the setting of acute biliary pancreatitis. In patients with cholangoitis, ERCP should be attempted as an emergency procedure. In evaluation of patients with idiopathic recurrent pancreatitis, noninvasive tests should be performed first. Endoscope ultrasound should preclude ERCP in most cases. Results of ERCP interventions in this setting are often equivocal. Endoscope procedures in the setting of ongoing severe pancreatitis are technically challenging and should be performed with extreme caution.
